Terms and conditions
English translation for information purposes. The clinic's original terms are available in Polish.
Dental practice
Quality Dent dental practice, operated by Izabella Skrzypczyk, provides healthcare services in Wroclaw at Wrobel Street 1. The practice does not provide services covered by Poland's National Health Fund (NFZ). These terms describe the organisation of healthcare services and apply to the practice's staff and patients.
Appointments
Appointments can be booked in person, by phone or through ZnanyLekarz.pl. Only the dentist determines how much time is needed for a procedure. Dental consultations are not provided over the phone or anywhere other than at the dental chair in the practice.
For urgent or painful conditions, the practice will try to offer the earliest available appointment. Patients receive a phone call or text reminder on the day before their appointment.
Patients with appointments have priority. The practice makes every effort to avoid delays, but procedures may take longer than expected. Patients are asked to allow for occasional delays of several minutes. Urgent cases are scheduled subject to availability.
First appointment
- Please arrive 10 minutes early to complete the necessary medical documentation.
- You will be asked to complete a patient form and provide consent for the processing of personal data.
- After examining you and carrying out any necessary additional tests, the dentist will present a proposed treatment plan and preliminary cost estimate.
Starting treatment
Before a procedure, the patient gives written consent. For prosthodontic treatment, the patient also accepts the cost estimate and treatment plan. The dentist may refuse to start or continue treatment if recommended procedures or tests have not been completed, such as specialist consultations, scaling, antibiotic treatment or diagnostic tests.
The practice may call to confirm an appointment on the preceding day.
Cancelling appointments
Please cancel an appointment at least one day in advance. An appointment involving implant placement must be cancelled at least three working days in advance. If a patient has missed three or more appointments, the clinic reserves the right to refuse or discontinue treatment.
Patient rights
While receiving healthcare services at Quality Dent, patients have the right to:
- reliable information about their health;
- respect for their dignity and courteous treatment by medical staff;
- healthcare provided by staff whose qualifications meet current medical standards.
Patients decide whether to consent to or refuse proposed healthcare services after receiving information about the proposed treatment method and necessary diagnostic tests. Information about patient rights is made available to patients.
Patient responsibilities
Patients are expected to follow these terms and their dentist's recommendations, maintain personal hygiene, observe the prohibition on smoking and consuming alcohol on the premises, follow safety and fire-safety rules, and carry valid identification documents.
Warranty and complaints
The Polish-language terms refer to the warranty provisions in Articles 627–646 of the Polish Civil Code. The practice states that, if during the warranty period a filling or prosthetic restoration becomes deformed or changes colour or properties, it will be repaired or replaced free of charge, subject to the exclusions below.
A complaint will not be accepted in cases including:
- an alternative rather than the recommended treatment plan was carried out at the patient's request;
- the patient discontinued the planned treatment or treated the teeth at another practice at the same time;
- the patient did not follow recommendations concerning oral hygiene, removable dentures or diet, including after whitening;
- the patient did not have a tooth restored after root canal treatment;
- dentures were used or stored contrary to the dentist's instructions;
- the prosthetic restoration was adjusted, altered or repaired by the patient or another practice;
- an accident caused mechanical damage to the restoration;
- bruxism, progressive bone loss causing a denture to settle, or damage to a denture outside the mouth;
- smoking, or chemotherapy or radiotherapy for cancer, in the case of implants and bone-regeneration procedures.
Possible complications
Despite due care, treatment may be followed by complications that are not covered by a complaint, including pain in the treated tooth or surrounding tissues, limited jaw opening, a cracked or fractured tooth, soft-tissue pain, swelling, an abscess, a haematoma or prolonged bleeding after surgery, fever or feeling unwell, an allergic reaction to medication, worsening of existing conditions, or tooth sensitivity after treatment or whitening.
These complications may cause discomfort and stress, require medication that is relevant to drivers or pregnant women, affect eating, speaking or appearance, or make further root canal treatment or extraction necessary. Most occur infrequently and resolve quickly, but the practice advises against scheduling major dental procedures shortly before important events such as a holiday or family celebration.
Fees and payment
Payment is due after each appointment according to the current price list. For prosthodontic treatment, a deposit of 30% of the total restoration value is payable at the first prosthodontic appointment. The balance is payable at the final appointment when the restoration is fitted.
The clinic may request a deposit for certain procedures seven days before the planned appointment, for example for implant placement or extensive prosthodontic work.
Medical records
Medical records are maintained electronically and in a paper patient record. They may be inspected at the practice after submitting an appropriate request, or made available as photocopies, copies or extracts. The patient or legal guardian must submit a written request.
The request must include the patient's full name, PESEL number (Polish national identification number) and handwritten signature. A patient who cannot collect the records in person may authorise another person in writing. The authorisation must include that person's full name, PESEL number and identity document number.
Records are prepared without undue delay and within seven days of an oral or written request. Patients are notified by phone when the records are ready. Records are released after identity is verified. They may be collected in person by prior arrangement; they cannot be sent by email or post.
Personal data and security
The practice applies a Personal Data Security and Processing Policy. The Polish-language terms identify the data controller as Quality Dent dental practice, with a registered address at 12 Ignacego Chrzanowskiego Street, 51-146 Wroclaw. The practice premises are under continuous audiovisual monitoring, except for sanitary facilities. Patients are responsible for damage to equipment and furniture. The clinic is not responsible for unattended belongings left on the premises.
Final provisions
Matters not covered by these terms are governed by applicable Polish law, including the Act of 15 April 2011 on Medical Activity and the Act of 6 November 2008 on Patients' Rights and the Patient Rights Ombudsman.
According to the Polish-language terms, these rules have applied at Quality Dent since 17 April 2024.
